WATER RELEASE FORM

PART I: The Envisioning Youth Empowerment Retreat may provide an opportunity for attendees to access bodies of water as part of planned group activities.  These bodies of water may or may not have lifeguards.  I authorize the Envisioning Youth Empowerment Retreat and its representatives to allow participation in swimming and water activities.

PART II: I agree that Envisioning Youth Empowerment Retreat and its representatives are not responsible for any injuries, actions, or decisions made while participating in swimming or water activities.
